The primary role of the Admissions Coordinator is to establish, develop, and maintain excellent referral relationships with referring physicians and discharge planners at targeted acute care facilities. The Admissions Coordinator is also responsible for the admission process and inpatient census of unit. Performs on-site patient evaluations to determine rehabilitation potential, level of care required and to provide information regarding RIC and its scope of services

Principal Responsibilities
1. Serves as a representative of RIC to referring physicians and discharge planners in order to identify their needs for rehabilitation services, increase their satisfaction with RIC and therefore, improve referral relations. 2. Acts as a resource to all referring parities regarding available services at RIC and Continuum. Establishes and maintains excellent relationships with all referring facilities, physicians, and associated clinical staff. 3. Maintain an up-to-date comprehensive understanding of the rehabilitation market in the assigned region including a detailed understanding of the rehabilitation competitors. 4. Performs on-site patient evaluations to determine rehabilitation potential, appropriate level of care, and determination of patient''s nursing needs. Confers with patient and family to determine their goals expectations for rehabilitation and provide orientation as appropriate. 5. Assess medical stability and nursing needs of inpatient referrals through the intake channel and communications to the RIC physicians and associated clinical staff. 6. Maintains responsibility for coordination of special patient services on a pre-admission basis such as dialysis, chemotherapy, radiation, special equipment needs, nutritional needs, etc. 7. Monitors correct assignment of patient types, hospital service codes and ICD (code assignments). 8. Acts as a resource to Third Party Payors and provides findings from a patient on-site evaluation as requested. Includes pre-admission development of patient goals for rehabilitation and negotiation and pre-certification of initial approved length of stay 9. Provides timely and appropriate completion of all pre-admission documentation with prompt and complete communication to nursing units. 10. Determines clinical appropriateness of patients and supports admission process by gathering and transferring medical information to the appropriate inpatient admitting team. 11. Collaborates with Director, Inpatient Admitting in the assessment of acute care hospitals/facilities. 12. Maintains excellent communication and relations between the Admitting Department and other internal departments that entail frequent interaction (eg. Patient Financial Services, Pharmacy, Central Supply, Scheduling, Institute Physicians/Residents, Institute nursing staff, Institute clinicians).
Knowledge, Skills & Abilities Required 1. Bachelor''s of Science (BSN) from an approved nursing program 2. Current license as a Registered Nurse as required by the State of Illinois. 3. At least one year of rehabilitation nursing experience in order to display a full knowledge of rehabilitation and health care delivery systems. 4. Must exhibit excellent knowledge of the organization, display confidence in patient assessment skills and communicate thorough understanding or rehabilitation medicine and its goals as related to patient care and outcomes. 5. Excellent interpersonal skills necessary to interact effectively with physicians, clinicians, insurance representative, patients/families, and a wide variety of potential referrers. These skills are necessary to effectively represent within the community. 6. Analytical skills necessary to develop plans, goals, and procedures in order to solve complex administrative and referrer/patient problems as well as maintaining a key sensitivity to payor considerations. 7. Valid driver''s license.
Working Conditions
1. Works routinely in patient care and office environments where there are minimal physical discomforts due to temperature, noise dust, and the like. 2. Must be willing to travel to other health care facilities using own vehicle when RIC vehicle or public transportation is not feasible. Some exposure to weather conditions and traffic
