Care Managers provide support to patients and families at RIC in order to facilitate successful adjustment to disability. They are responsible for psycho-social assessment, discharge planning, and education around community services/entitlements. In collaboration with the rehabilitation team, Care Managers monitor the interdisciplinary plan of care, the patient''s goals and available resources in order to maximize cost-effect treatment and ensure a smooth transition throughout the continuum.
Care Managers consistently demonstrates support of the RIC statement of Mission and Philosophy by striving for excellence, contributing to the team efforts and showing respect and compassion for patients and their families, fellow employees, and all others with whom there is contact at or in the interest of the institute.
Care Managers demonstrates RIC Core Attributes: Communication, Accountability, Flexibility/Adaptability, Judgment/Problem Solving, Customer Service and RIC Values (Hope, Compassion, Discovery, Collaboration, & Commitment to Excellence) while fulfilling job duties.
Principal Responsibilities
1. Completes psycho-social assessments to gather pertinent information for the team regarding patient adjustment, family supports, financial resources, housing, understanding of disability and barriers to discharge. 2. Facilitates team conference and interdisciplinary documentation. 3. Coordinates the interdisciplinary discharge plan. Addresses community needs including follow-up medical appointments, ongoing therapy/rehabilitation, attendant care, short & long-term placement, transportation, DME/Disposable Supplies, medications, home assessments, income maintenance and family support services. 4. Serves as a resource to families experiencing difficulty with violence/abuse, poverty, mental illness, substance abuse, cultural/language barriers and other social challenges that may impede progress in rehabilitation. 5. Monitors patient and family goal attainment relative to the interdisciplinary treatment plan and addresses the appropriateness of the level of care during clinical team conferences. 6. Acts as a resource regarding available services at the Rehabilitation Institute of Chicago System of Care. 7. Initiates and maintains communication with external case managers, third party payers and primary care physicians to obtain authorization for ongoing rehabilitation needs and ensure reimbursement for RIC services. Advocates for the patient to receive appropriate rehabilitation services. 8. Negotiates any conflict arising with a third party payor representatives and supports the RIC UR process by initiating formal appeals on behalf of the patient and Institute during their rehabilitation program. 9. Participates in planning, development and execution of Rehabilitation Institute of Chicago sponsored educational programs and presentations to external physicians, clinical staff, acute care facilities, insurance groups, managed care organizations, corporations, and other appropriate groups.

1. Reports to the Director of Care Management and Family Support Services
Knowledge, Skills & Abilities Required
1. Work requires a professional level of knowledge in a specialized field of rehabilitation. Nursing care managers must have 2 years experience in rehabilitation nursing and have CRRN certification. Social work care managers must hold an MSW. 2. Illinois license in one''s own profession. 3. Knowledgeable community resources, health care system and interdisciplinary functioning. 4. Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the Institute. 5. Capable of learning and utilizing PC software to collect and analyze data and prepare and aggregate reports.
Working Conditions
1. Normal patient environment with little or no exposure to dust or extreme temperature. 2. Occasional exposure to infectious diseases when working with patients.
