Part-time Physical Therapist/NeuroRestorative/Des Plaines, Illinois Founded in 1977, NeuroRestorative was the first dedicated after-hospital rehabilitation program in the country for persons with acquired brain injuries and has continued as a leader in the field for more than 30 years. Position Summary: To provide program participants evaluations, and physical therapy services. Key Responsibilities:
Evaluate program participants through appropriate evaluation procedures and develops written treatment programs for each participant.
Maintains timely and appropriate program participant/administrative and department documentation.
Ensures adherence to CARF, licensure standards and ethics of confidentiality.
Writes weekly service summaries and maintains daily billing summaries and turns into appropriate staff
Requirements:
Bachelors Degree in Physical Therapy
One years experience preferred
State Licensure in Physical Therapy
Current Valid Illinois Drivers License
Acceptable Driving Record
Ability to pass drug screen and background checks
